Step 3: Participate in Wellness Actions by August 31, 2013

Following completion of the HWA, you can earn up to $100 in additional Wellness Rewards. Each Wellness Action earns $50. These may include:

  • Completion of the HWA by a spouse/partner enrolled in your D-H health insurance
  • Online digital coaching program and 30-day follow-up at www.my.healthmedia.com
  • LWWW personalized health/lifestyle coaching and 30-day follow-up
  • Attendance at other health and wellness activities, courses and workshops in the following categories:
    • Personalized tobacco cessation
    • Nutrition education
    • Weight management
    • Stress management
    • Sleep hygiene
    • Fitness activity

The activity, course or workshops must total four hours. You may choose to participate in more than one program to accumulate these four hours. Any individual activity that is more than four hours in length meets the requirement of one Wellness Action, even when the total hours are eight or more.

Health Plans, Inc. is partnering with us in the administration of this program.

You must complete and submit the Wellness Rewards Program Activity Verification form to receive credit for these wellness actions. This form is located on the Health Plans, Inc. website at www.healthplansinc.com/D-H. You also can track your activities on this website and access other wellness resources. Note: You are not required to submit this form for Health Media activities (completion of HWA by spouse/partner and/or online digital coaching program and 30 day follow-up).

Wellness Action(s) completed by May 31st will be verified and paid to benefit-eligible active employees in the July 26th (bi-weekly) or July 31st (monthly) paycheck. You must be a benefit-eligible active employee on July 7th (bi-weekly) or July 1st (monthly) to receive the taxable reward credit.

Wellness Actions completed between June 1st and August 31st will be verified and paid to benefit-eligible active employees on October 4th (bi-weekly) or 31st (monthly). You must be a benefit-eligible active employee as of September 15th (bi-weekly) or October 1st (monthly) to receive your reward payment for Wellness Actions completed between June 1st and August 31st.

The maximum credit for Wellness Actions is $100.00.
