Total Worker Health Care
Think of Live Well/Work Well Primary Care as providing total worker health care, focused on your needs as a worker and integrating all the resources that D-H makes available to support your health. Live Well/Work Well Primary Care offers different ways to connect with your care team. For example, a member of your care team may wish to informally (and by your request) visit you at work to better understand how your work can affect your health and your specific health needs. By visiting you at your work site, your team will have a better understanding of the demands of your job as well as your overall health and be able to recommend D-H resources that may help support you. Like all D-H primary care services, we are committed to giving you the tools and information you need to keep you healthy and help you manage chronic conditions as needed.
All Dartmouth-Hitchcock employees have access to the full range of Live Well/Work Well services whether or not they are insured. If you already have an established relationship with a D-H primary care provider, you will continue to have access to all Live Well/Work Well resources. These services include:
- Health and wellness assessments
- Biometric (health) screenings
- Diet and nutrition counseling
- Tobacco cessation program
- Behavioral/mental health counseling
- Exercise programs
- Occupational health program
- Care Coordination for employees on the group health plan
Live Well/Work Well Primary Care members have access to all of the Live Well/Work Well resources in addition to a dedicated primary care team for:
- Annual wellness appointments
- Chronic condition management
- Referrals to specialists
- Preventive screenings
- Urgent/sick care appointments
If you are insured through Dartmouth-Hitchcock, there is no cost difference in signing up for Live Well/Work Well Primary Care versus other primary care services at Dartmouth-Hitchcock.
Keeping Your Health Information Private
All of the providers who are part of the Live Well/Work Well Primary Care team are part of the Dartmouth-Hitchcock group practice and use our electronic health record system, eD-H, to document your care, maintain your medications and provide care coordination.
Your privacy is of the utmost importance to us. We strictly abide by all HIPAA regulations, keeping all of our patients’ information private. We will not share your health information with third parties or employers without your written consent.
